You will need:
Paintshop Pro - I used 9 but can be done in earlier versions
Tube of choice or get the one I used from Outlaw by Design
Font I used HERE

Unzip the font and open and minimise it
Open your tube in psp
Open a new transparent canvas 600 x 400
Copy and paste your tube as a new layer
Resize as necessary, making sure All Layers is NOT ticked
Select 2 colors from your tube
A light color for the foreground/stroke and a dark color for the background/fill
Grab your text tool with the following settings:
Text Settings
Type your name and apply
Now stretch your text up and down to almost the same height as
tube and place it to the left of canvas (see my finished tag)
(You may need to to make your name narrower too if it is longer then mine)
Text Layer
When happy Convert to Raster Layer
With this text layer selected go to
Selections - Select All - Selections Float - Selections Defloat
Add a new raster layer then paste tube into selection
Select None
Add the following Drop Shadow to this layer
V = 2 H = -2 (minus) Opacity = 65 Blur = 2 Color = black
Repeat this drop shadow on text layer and tube layer
Turn the opacity of the text layer down to 50 or so
Bring the tube layer to the top and move it into place on the far right of canvas
You only want it to cover a small part of your name
If happy with placement, Merge Visible
New Raster Layer - Move it to the bottom of layer palatte
Flood fill with your light color
Now crop your tag like so without cropping any of your name
Leave small space around name
On the bottom layer go to Effects - Texture Effects - Blinds:

The color I used was the a few shades darker then the
light color in foreground
To darken this color just reduce the number in the L box by 10-15
Making Colors Darker
Repeat Blinds but untick the Horizontal box this time
If you would like to add some brushes,
add a new layer and use your favorites
When happy Merge All Flatten
Image - Add Border - 6 pixels Color= #ffffff (white) and symetric ticked
Select border with your magic wand
Now we need to make our foreground color even lighter
Do this by increasing the number in L box by 25 or more
making your color lighter
Floodfill border with this new lighter color
Selections - Invert
Drop Shadow V & H = 2 Opacity = 65 Blur = 2 Color = black
Repeat drop shadow but change the V & H to -2 (minus 2)
Select None
Resize your tag to your liking
Add copyright and watermark etc and save
